Merck’s antiviral could be just what Covid was waiting for

‘There's a possibility that SARS-CoV-2 could do an end run around the drug; any variant that happens to be less susceptible to the drug’s modus operandi could survive and become more dominant, pushing the virus’s evolution toward resistance. And SARS-CoV-2 has already shown its propensity for outsmarting certain treatments: In July 2021 the US halted shipments of an antibody therapy from Eli Lilly after detecting resistance in newer variants of the virus.’

The raging evolutionary war between humans and Covid-19

‘Fighting the pandemic isn’t only about vaccines and drugs. It’s about understanding how viruses mutate and change inside us, and among us...

‘The major change to the immunity of all the hosts SARS-CoV-2 is likely to try to infect will be, of course, vaccination. That’s human ingenuity fighting viral expertise, but it can also exert a kind of direct adaptive pressure on the virus. History has examples of so-called leaky vaccines—those that aren’t effective enough to prevent all infections or all transmission, and allow better-adapted variants of whatever bug they’re trying to squish to live to fight another day.

‘In fact, one group of researchers has a model that suggests that could even happen with the new batch of vaccines against Covid—especially those that require two doses and seem to confer different levels of immunity depending on how far apart they’re administered, or whether someone skips the second one. Here's how: If one extreme is a population totally naive to a new virus, completely vulnerable and with no immunity, and the other extreme is a population with perfect sterilizing immunity, what happens to a population in between? If a vaccine allows infection but no transmission, the virus doesn’t have a chance to evolve.

‘But if a vaccine or vaccination strategy allows some infection and some transmission? “The ones that are the best at getting around the host’s defenses are the ones that are most likely to persist,” says Caroline Wagner, a bioengineer at McGill and one of the people working on the model. If that’s all true, a leaky vaccine or leaky vaccination strategy could actually drive antigenic drift and create even worse variants. Wagner and her colleagues acknowledge that they don’t have enough data to put bounds on their model yet, but they worry about strategies like one proposed in the UK to abandon second doses as a way of speeding the process and husbanding scarce vaccine, or the way some countries are hoarding vaccine while others go without (potentially letting the virus, and variants, circulate and evolve freely).’

More Covid vaccine choices mean new equity challenges

‘More questions about priority are likely as the distribution effort matures, says Julie Swann, a vaccine supply chain expert at North Carolina State University. Soon, there should be more than three vaccines—shots from AstraZeneca and Novavax are likely next—and each will come with pros and cons. Even if shots are eventually targeted for one group or another, transparency will be key, she notes. “You can tell people that one is less effective but you wouldn’t have to come back, and the other is more effective but you need a second dose,” she says. Some people may have preferences between one or the other. Maybe some will seek out a different shot elsewhere. Maybe some will wait. But most, she hopes, will see little choice in the range of good options. In the middle of a crisis, a shot is a shot.’

‘Uncomfortable relatives are my specialty. As a doctor, I’ve been trying for years to get my family and friends to follow prudent medical advice. It hasn’t gone so well. A decade ago, they’d say, “You’re just in medical school.” Then I was “just in residency.” Now, unfortunately, I’m a pathologist, which doesn’t exactly scream “people person.” I have had some successes, though. Last year, after a hundred gentle conversations, I finally convinced my parents to get the flu vaccine. (Verdict: “It wasn’t so bad.”) A couple months later, Covid-19 struck. I try not to feel completely responsible for this irony...’

Winning trust for a vaccine means confronting medical racism

‘Just about every minority group residing in the United States can point to what feels like a reasonable basis for suspicion. For African Americans, there is not only the notorious Tuskegee study, which withheld syphilis treatment from rural Black men, but also experiments that used enslaved women to perfect surgical techniques and studies that tested new drugs in poor neighbourhoods without adequate consent. The Latino community can point to a syphilis study in Guatemala that was even more unethical than the Tuskegee one, and to pharma companies basing tests of the first versions of birth control pills, which caused significant side effects, in Puerto Rico (and also in Haiti). Attempting to pass smallpox to Native Americans via contaminated blankets is an infamous episode in Colonial-era history, and the US government has underfunded the Indian Health Service since its 1955 founding, depriving reservation dwellers of what should have been guaranteed medical care.’

The analogy between Covid-19 and climate change is eerily precise

“We went through the stages of climate change denial in the matter of a week,” said Gordon Pennycook, a psychologist at the University of Regina in Saskatchewan, Canada, who studies how misinformation spreads. Naomi Oreskes, a historian of science who has studied the origins of climate disinformation, spelled out the pattern in an email: “First, one denies the problem, then one denies its severity, and then one says it is too difficult or expensive to fix, and/or that the proposed solution threatens our freedom.”
