Right to life: Dead in India?

‘Constitution and national health policies should have been the guiding philosophy for leaders and planners. But did we even read them?... The deadly combination of uneducated leaders, greedy corporates, ignorant public, and the contemptuous disregard for science has wreaked havoc. Our priorities are misaligned and actions ill-intentioned.’

PM Modi owns India’s unfolding Covid disaster: Bloomberg

‘As in so many of the pandemic’s worst-hit countries, this tragedy was avoidable — and is largely the fault of a boastful and incompetent government. Yet, judging by the fate of other bungling far-right politicians such as Brazil’s Jair Bolsonaro, the UK’s Boris Johnson, Hungary’s Viktor Urban, and the Philippines’ Rodrigo Duterte, Indian Prime Minister Narendra Modi may well suffer few political consequences for his devastating missteps...

‘Yet, while Trump lost to Joe Biden in large part because of his callous and clueless handling of the pandemic, the margin was disturbingly narrow. Other strongmen look more likely to survive politically — and to continue to add to the toll of needless deaths.

‘For his part, Modi not only enjoys much higher approval ratings than Trump ever did. He has also survived, already, blunders that would have wrecked any other political career: demonetization in 2016 and a botched lockdown last year that caused the biggest and most desperate internal migration witnessed in India since 1947.

‘Modi has flourished with the help of something Trump never had and the likes of Boris Johnson only sporadically enjoys: a compliant media. Indeed, one reason why complacency about the virus spread so widely in India is that Modi personally asked owners and editors of press and television in March last year to focus on “positive” stories. Evidently, as his website put it, “it was important to tackle the spread of pessimism, negativity and rumor.”

Covid: The countries that nailed it, and what we can learn from them

‘I have reported on Covid for the past year - now my mission was to find out from global leaders and senior health officials across four continents what their priorities were in tackling the virus.

‘What has emerged strongly for me are four key areas which have been most effective in containing the spread of the virus and preventing deaths.

  • Early and effective action to control borders and monitoring of arrivals
  • Testing, tracking and tracing everyone suspected of being infected
  • Welfare support for those in quarantine to contain the virus
  • Effective leadership and consistent and timely public messaging

Covid-19: Social murder, they wrote — elected, unaccountable, and unrepentant

‘After two million deaths, we must have redress for mishandling the pandemic... Murder is an emotive word. In law, it requires premeditation. Death must be deemed to be unlawful. How could “murder” apply to failures of a pandemic response? Perhaps it can’t, and never will, but it is worth considering. When politicians and experts say that they are willing to allow tens of thousands of premature deaths for the sake of population immunity or in the hope of propping up the economy, is that not premeditated and reckless indifference to human life? If policy failures lead to recurrent and mistimed lockdowns, who is responsible for the resulting non-covid excess deaths? When politicians wilfully neglect scientific advice, international and historical experience, and their own alarming statistics and modelling because to act goes against their political strategy or ideology, is that lawful? Is inaction, action?1 How big an omission is not acting immediately after the World Health Organization declared a public health emergency of international concern on 30 January 2020?

‘At the very least, covid-19 might be classified as “social murder,” as recently explained by two professors of criminology. The philosopher Friedrich Engels coined the phrase when describing the political and social power held by the ruling elite over the working classes in 19th century England. His argument was that the conditions created by privileged classes inevitably led to premature and “unnatural” death among the poorest classes. In The Road to Wigan Pier, George Orwell echoed these themes in describing the life and living conditions of working class people in England’s industrial north. Today, “social murder” may describe the lack of political attention to social determinants and inequities that exacerbate the pandemic. Getting redress

‘Where then should citizens turn for accountability, if they don’t find it in their leaders and feel unsupported by experts and the media? The law remains one form of redress, and indeed some legal avenues, including criminal negligence and misconduct in public office, are being explored, although proving any such claims will be difficult and drawn out. But the notion of murder, at least “social murder,” is hard to shake emotionally, and strengthens with every denial of responsibility and every refusal to be held accountable or to change course.

‘That leaves three options. The first is to push for a public inquiry, as The BMJ and others argued for in the summer of 2020—a rapid, forward looking review rather than an exercise in apportioning blame that will identify lessons and save lives. The second is to vote out elected leaders and governments that avoid accountability and remain unrepentant. The US showed that a political reckoning is possible, and perhaps a legal one can follow, although research suggests that mishandling a pandemic may not lose votes.21 The third is for mechanisms of global governance, such as the International Criminal Court, to be broadened to cover state failings in pandemics...

‘The “social murder” of populations is more than a relic of a bygone age. It is very real today, exposed and magnified by covid-19. It cannot be ignored or spun away. Politicians must be held to account by legal and electoral means, indeed by any national and international constitutional means necessary. State failures that led us to two million deaths are “actions” and “inactions” that should shame us all.’

Is Noor Hisham a bystander in the Covid-19 crisis? Frankie de Cruz

‘Some claim the director-general of health, who wasn’t fazed by whatever was thrown at him in the early days of the crisis, is in a political straitjacket. Could it be that his views have narrowed the political and economic choices of those in power to relatively tight parameters?

‘If that is true, it needs fixing. In the meantime, the people expect professionalism and strong leadership at a time when Covid-19 infections and deaths are rising. That means Noor Hisham has to be firm and shoot straight to give Malaysians the confidence that the war is being fought with confidence.’

Dying in a leadership vacuum: NEJM editorial

‘Covid-19 has created a crisis throughout the world. This crisis has produced a test of leadership. With no good options to combat a novel pathogen, countries were forced to make hard choices about how to respond. Here in the United States, our leaders have failed that test. They have taken a crisis and turned it into a tragedy.

‘The magnitude of this failure is astonishing. According to the Johns Hopkins Center for Systems Science and Engineering,1 the United States leads the world in Covid-19 cases and in deaths due to the disease, far exceeding the numbers in much larger countries, such as China. The death rate in this country is more than double that of Canada, exceeds that of Japan, a country with a vulnerable and elderly population, by a factor of almost 50, and even dwarfs the rates in lower-middle-income countries, such as Vietnam, by a factor of almost 2000. Covid-19 is an overwhelming challenge, and many factors contribute to its severity. But the one we can control is how we behave. And in the United States we have consistently behaved poorly.’

Covid-19: Protecting our children ― Amar-Singh HSS

‘This recent infant death will have alarmed parents and those of us working with children. We want to know how we can protect our children better. The clear message is that we cannot protect our children without controlling the pandemic in the community; these two are intricately linked. I would like to offer some suggestions of what we can do in the face of the extensive community spread that is currently occurring in the country.’

  • Improve SOPs at schools, taskas, child care facilities
  • Strengthen our contact tracing with data transparency
  • Advocate for health support to be ramped up with regards to (1) testing (2) use of rapid antigen detection tests (3) boost of manpower at MOH (4) injection of funds at MOH for equipment, PPEs, etc
  • Stop poor leadership by example that hampers population compliance

A world in disorder

On September 14th 2020, the GPMB released its second report titled, A World in Disorder. In this report, the GPMB provides a harsh assessment of the global COVID-19 response, warning that the world cannot afford to be unprepared again when the next pandemic hits. The Board called for five urgent actions to be taken to bring order out of the catastrophe and chaos currently facing the world: responsible leadership; engaged citizenship; strong and agile systems for health security; sustained investment; and robust global governance of preparedness.

The Global Preparedness Monitoring Board (GPMB) is an independent monitoring and accountability body to ensure preparedness for global health crises. Comprised of political leaders, agency principals and world-class experts, the Board provides an independent and comprehensive appraisal for policy makers and the world about progress towards increased preparedness and response capacity for disease outbreaks and other emergencies with health consequences. In short, the work of the GPMB will be to chart a roadmap for a safer world.

It is wide ranging, querying the changing role of international cooperation and the fallout of austerity economics, and taking a deeper dive into China’s scientific and political response to the crisis than most Western media have offered. But the book returns again and again to the catastrophe in both the United Kingdom and the United States. It is haunted by the question: how did two of the richest, most powerful and most scientifically advanced countries in the world get it so wrong, and cause such ongoing pain for their citizens?’

Merck CEO Ken Frazier discusses a Covid cure, racism, and why leaders need to walk the talk

‘As chairman and CEO of the leading vaccine producer in the world, pharmaceutical giant Merck & Co., Ken Frazier has one of the highest-profile positions in global business.

‘But Frazier, who is leading one of the firms on a charge to develop a COVID-19 vaccine, is unique in another way: He is just one of four Black CEOs leading a Fortune 500 company. Frazier is also outspoken, having resigned from President Trump’s American Manufacturing Council to make a clear statement against “hatred, bigotry and group supremacy” that surfaced in protests at Charlottesville, Virginia.

Scathing Covid-19 book from Lancet editor — rushed but useful

‘Since the coronavirus crisis began, Richard Horton, editor-in-chief of leading medical journal The Lancet, has been tearing across the British public sphere. Here he is on the BBC, the national broadcaster, there in the pages of The Guardian newspaper — taking the government to task for failures that have left the United Kingdom with the world’s second-highest per capita COVID-19 death toll so far (Belgium is top). Horton has never shied away from controversy (his journal published the retracted, fraudulent paper by Andrew Wakefield that alleged a non-existent link between vaccines with autism) or crusades (against the Iraq war and for political action on climate change). In coronavirus, he has found a cause that matches his energy: the Lancet journals are pumping out both the latest research and his pointed critiques of government policy; and last month, he reviewed a new book by the Slovenian Marxist philosopher Slavoj Žižek that imagines economic and social worlds after COVID-19.

Harnessing our humanity — How Washington’s health care workers have risen to the pandemic challenge

‘Given mounting PPE shortages and the rising death toll from Covid-19 among health care workers globally, such scenarios [of protocol modifications -- including the use of bandannas in place of face masks] induce terror and necessitate rethinking of routine standards of care. Should we run to codes of patients with known or suspected Covid-19? Should physicians refuse to intubate a patient unless they have adequate PPE? Should we offer bedside comfort to the elderly patient with possible Covid-19 who becomes increasingly delirious as unrecognizable health care workers speed in and out of the room while TV coverage of the pandemic drones on in the background? The clarion call of the profession — to put our patients’ needs above our own — is far less directive when meeting one patient’s needs may leave us too ill to care for the next.’

Questions about leadership and open-mindedness: A response to why Covid-19 deniers stick to their beliefs

‘Q: What kind of leadership is needed right now, given our belief systems and what is at stake?

‘A: We all need to be more intellectually humble. We all need to recognise that how certain we feel is irrelevant to how certain we should be. We need to recognise that there are scientists and medical experts out there who have the knowledge and expertise we need to make smart decisions, and they are willing and able to share that information with us.

‘We need our leaders especially right now to understand the role they have in all of this. Words aren’t just words. Words are the basis of beliefs, and beliefs drive our behaviour. People who don’t believe the pandemic is real or that it will spread put themselves and everyone else at risk by not doing what needs to be done to stop it…

‘Q: Can people be taught to be more open-minded?

‘A: It’s possible. Beliefs are not stable, finished things because we are constantly taking in new data and updating. It may seem disheartening that people shut down once they become certain. But I see hope in the fact that people are fundamentally social and that they seek to engage with one another.

‘People are sensitive to the beliefs of those around them. When those beliefs change, people may reconsider their positions. That’s why talking about what is happening is important, and informed people who know the most should be talking the loudest.’
